Forms and surveys

Forms are what drivers fill in — the check-in and check-out with their walk-round photos, an incident report, anything you build — and surveys are what you ask. Both are made in the same builder, answered on the driver’s phone, and read back on the office’s screens.

The forms you start with

Forms

Every company starts with the check-in and check-out, the walk-round photo positions already laid out, and the incident and accident forms the board sends. The list shows each form’s type, its version and how many have answered; a form is Live, Retired or Edited.

Building or changing a form

  1. Press + New form, or Edit on a form.

    The builder: the Parts bin, then Questions, Rules and a Phone preview. A photo question asks for A van position — and where the driver stands for it — or Anything.

  2. Say what an answer does.

    A rule can Flag the submission, Notify the ops team, or Send them another form.

  3. Publish it.

    Publish as vN. The old version stays, so an answer from last month still reads against the words that were on the screen then.

Sending a form

Pick the form and Who gets it — specific drivers, every driver at the station, everyone working that day, or each driver after their first route completes — an optional close date, and Assign. The incident form goes from the board instead: Send incident form on a driver’s row, and the driver’s pick files the van’s damage record, an injury report, or both.

Flags

On the board
The flags strip under the day: what the forms raised today, and who has seen it.

Every flag, open or seen. Mark seen when it is handled.
Settings › Flag words
The word each check shows on the board, and which of them are red.

Surveys

Forms › Surveys › + New survey

  1. Write the questions.

    The same builder — then Save and choose who gets it.

  2. Choose who, and how.

    Once, or Every two weeks, on a day they work; One page or One question at a time; Everyone, the Office team, Drivers or Pick people; and whether answers show Names or No names.

  3. Send it.

    Send survey to N people asks first, because every phone it reaches rings. An every-two-weeks survey starts with Start — every two weeks and asks each driver after a check-out, round after round, until you stop it; its questions are fixed once it starts.

  4. Read the results.

    % favorable per question, who has not answered, Remind the N, and Close survey or Stop the survey.
